Services


ONLINE APPLICATION OF HONORABLE DISMISSAL (HD), CERTIFICATE OF NON-ISSUANCE OF HONORABLE DISMISSAL (NIHD), AND OTHER CERTIFICATES

  1. Student downloads and fills out the Application Form (click here to download);
  2. Students emails the filled-out Application Form to rmaschief.ourdiliman@up.edu.ph;
  3. Records Management Section (OUR-RMS) replies with a Payment Slip;
  4. Student pays via fund transfer feature of his/her bank to Landbank or pays over-the-counter (OTC) at any open Landbank branch (Note: If paying via fund transfer, please use PESONet);
  5. If applicable, student waits for the confirmation email from the bank that the fund transfer is successful (proof of payment);
  6. Student emails the photo/scanned copy of the proof of payment and the filled out Payment Slip to rmaschief.ourdiliman@up.edu.ph;
  7. If everything is in order, OUR-RMS prepares and prints the document requested;
  8. OUR notifies the student if the document is ready for pickup; and
  9. Student claims the documents at the OUR-RMS.
  10. Note : Processing time is three working days. Requesters are advised to only make a follow-up inquiry if the document requested went beyond the 3-day processing period. Thank you.

HOW TO APPLY FOR AN HONORABLE DISMISSAL

  1. Secure a University Clearance from the One-Stop Student Clearance Center of Admission and Registration Section. When cleared, get an order of payment slip at the Records Section. Pay P30.00 fee at the Cashier's Office, at the back of PNB.
  2. To claim, present receipt to RMAS after 3 working days.
  3. Note : Honorable Dismissal is issued only once. In case of loss, the current school of the student shall have the discretion to request for verification about the issuance of the said document.

HOW TO APPLY FOR A CERTIFIED TEXT OF DIPLOMA

  1. Secure an order of payment slip at the Records Section.
  2. Pay P40.00 /copy at the Cashier's Office at the back of PNB.
  3. After 3 working days, present receipt to claim and authorization letter when claiming certified text of another person.

HOW TO REQUEST FOR SCHOLASTIC VERIFICATION

  3. Pay at the Cashier's Office,at the back of PNB or thru Postal Money Order
    1. P100.00 for local verification
    2. US$30.00 for foreign verification. Payment may be done thru International Postal Money Order or Manager's Check, Cashier's Check, Traveller's Check or Company's Check
  4. Claim after 3 working days at the RMAS counter; receipt must be presented.
  5. Note : Requests through email can be replied through email also after payment is received by RMAS.
