Shifting Application to another Degree Program Within the College (Shifting 1)

  1. Student accomplishes application form and submits required documents to the Student Records Section on or before the deadline set by the college or school
  2. Submit the following requirements:
    • Accomplished Application Form
    • Official Receipt of Application Fee
    • True Copy of Grades
    • Certificate of Enrollment
    • Certificate of No Contract
    • Certificate of Good Moral Character
    • Other documents required by the college
  3. Student follow-up the result of initial evaluation on the date identified by the college or school.
  4. College SRE informs the accepted applicant of the results and issues the following:
    1. Notice of Acceptance
    2. List of requirements such as Permit to Transfer and College Clearance
  5. College SRE gives the Form 001or Change of Program to accepted applicant (S1) and two copies of Student Directory (to be accomplished by the student) to be forwarded to the ARS-OUR
  6. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. College Admission Slip (Form 001 Change of Program)
    2. Two copies of accomplished Student Directory
  7. Student submits the College copy of the University Admission Slip to the College SRE.

    • Certificate of No Contract

  3. Student follow-up the result of initial evaluation on the date identified by the college or school.
  4. College SRE informs the accepted applicant of the results and issues the following:
    1. Notice of Acceptance
    2. List of requirements such as Permit to Transfer and College Clearance
  5. College SRE gives the Form 001or Change of Program to accepted applicant (S2) and two copies of Student Directory (to be accomplished by the student) to be forwarded to the ARS-OUR
  6. Student submits the following to the ARS-OUR for issuance of University Admission Slip:

    2. Two copies of accomplished Student Directory
    3. Original True Copy of Grades used for evaluation duly stamped and signed by SRE or College Secretary
    4. Permit to transfer from the former College (original copy)

  1. Student accomplishes application form and submits required documents to the Student Records Section on or before the deadline set by the college or school
  2. Submit the following requirements:
    • Accomplished Application Form
    • Official Receipt of Application Fee
    • True Copy of Grades
    • Certificate of Enrollment
    • Certificate of No Contract
    • Certificate of Good Moral Character
    • Other documents required by the college
  3. Student follow-up the result of initial evaluation on the date identified by the college or school.
  4. College Secretary/Coordinator submits the list of qualified applicants to the ARS-OUR.
  5. College SRE informs the accepted applicant of the results and issues the following:
    1. Notice of Acceptance
    2. List of requirements such as Permit to Transfer, College Clearance and Referral slip to UPHS for Medical Certificate
  6. College SRE gives the Form 002 (College Admission Slip) to accepted applicant (T1) and two copies of Student Directory (to be accomplished by the student) to be forwarded to the ARS-OUR
  7. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. College Admission Slip (Form 002)
    2. Two copies of accomplished Student Directory with photos
    3. Original True Copy of Grades used for evaluation duly stamped and signed by SRE or College Secretary
    4. Permit to transfer from the former College (original copy)
    5. Medical Certificate from the UPHS
    6. Photocopy of PSA-Birth Certificate, original to be presented
    7. Photocopy of PSA-Marriage Certificate, original to be presented (if married woman)
    8. Study permit from OSA, if foreign student
  8. Student submits the College copy of the University Admission Slip to the College SRE

Transfer Application from Other Schools or Universities (Transfer 2 and Second Degree Applicants)

Note: UP Diliman accepts transfer students every first semester only

  1. Student accomplishes application form (OUR Form 3) and submits required documents to the OUR-ARS on or before the deadline set by the UR
  2. Submit the following requirements:
    • Application Form with photos
    • Official Receipt of Application Fee (Php100.00)
    • 4 Photocopies of True Copy of Grades (signed by the Registrar of your school/university, original to be presented
  3. Student follow-up the result of initial evaluation 10 days after the deadline of application period
  4. ARS-OUR SRE refers all qualified applicants to the Colleges
  5. Student follow-up the results in the Colleges he indicated in the Application form
  6. College Secretary/Coordinator submits the list of qualified applicants to the ARS-OUR.
  7. College SRE informs the accepted applicant of the results and issues the following:
    1. Notice of Acceptance
    2. List of requirements to be submitted to OUR-ARS and
    3. Referral slip to UPHS for Medical Certificate
  8. College SRE gives the Form 002 (College Admission Slip) to accepted applicant (T2) and two copies of Student Directory (to be accomplished by the student) to be forwarded to the ARS-OUR
  9. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. College Admission Slip (Form 002)
    2. Two copies of accomplished Student Directory with photos
    3. Honorable Dismissal/Transfer Credential
    4. Medical Certificate from the UPHS
    5. Photocopy of PSA-Birth Certificate, original to be presented
    6. Photocopy of PSA-Marriage Certificate, original to be presented (if married woman)
    7. Study permit from OSA, if foreign student
  10. Student submits the College copy of the University Admission Slip to the College SRE

Transfer Application of Foreign Students from Other Schools or Universities Abroad (Transfer 2 and Second Degree Applicants)

Note: The University of the Philippines Diliman accepts Foreign Transfer Student Applications during the First Semester only of each Academic Year

  • Deadline for foreign applicants to file their application for transfer is April 15.
  • Release of application results - May 15

Procedure

  1. Foreign applicant sends by post Application Form (OUR Form 3.1) together with the following requirements:
    1. Application form with passport size photo
    2. Application fee in PMO USD $30 or Manager’s check USD $30
    3. Transcript of Records for evaluation purposes ( for undergraduate applicants)
    4. Official Transcript of Record ( for second degree applicants )
    5. If the medium of instruction in a previous school is not English, TOEFL Result with any of the ff. scores: paper-based-500, computer based-173, IBT -61 or IELTS -5.5 is required.
    6. If there is a gap in enrollment, a Certificate of Non- enrollment is required stating the particular semester/AY that the student was not enrolled.
  2. OUR Admission staff evaluates and computes the GWA of the foreign applicant.
  3. If GWA meets the requirement of program applied for, OUR Admission staff will refer the application to concerned college/unit.
  4. OUR Admission staff informs the foreign applicant by e-mail that his/her application has been referred to the college/unit.
  5. The Office of the College Secretary (SRE) evaluates and forwards the application to the department concerned based on the ff:
    1. GWA
    2. Number of academic units taken from another university
    3. Determines courses for validation of advance credit
  6. The Department schedules the exam and interview of the student.
  7. The Department informs the student of the schedules for exam and interview. Contact number/e-mail address of foreign applicant is written on the referral slip prepared by the OUR and attached to the applicant’s transcript of record.
  8. The Department through the Office of the College Secretary should forward the result of the screening (exam and interview) on or before the last week of May to the Admission & Registration Section of the OUR. The Office of the College Secretary issues a College Admission Slip.
  9. The OUR then prepares the Acceptance Letter to be sent by post to the foreign applicant. A copy is also forwarded to the International Students’ Program.
  10. OUR notifies the applicant of admission by email. The OUR informs the applicant of the requirements needed for enrolment. A scanned copy of his/her Acceptance Letter is sent via e-mail with instructions that he/she has to get in touch with Miss Esther Mendoza of the International Students’ Program of the Office of Student Affairs regarding the processing of student visa.
  11. Accepted Foreign Applicants are advised to arrive in the Philippines at least 2 months before registration time to process their student visa. They will be assisted by Ms. Mendoza of the International Students’ Program of the Office of Student Affairs.

Graduate Application (for local applicants)

Note: For MA, PhD, Diploma Program and Non-Degree applicants

  1. Student accomplishes application form and submits required documents to the Student Records Section on or before the deadline set by the college or school
  2. Submit the following requirements:
    • Application Form
    • Official Receipt of Application Fee
    • Official Transcript of Records
    • Photocopy of PSA-Birth Certificate, original to be presented
    • Photocopy of PSA-Marriage Certificate, original to be presented (if married woman)
  3. Student follow-up the result of evaluation on the date identified by the college or school
  4. College SRE informs the accepted applicant of the results and issues the following:
    1. Notice of Acceptance;
    2. List of requirements to be submitted to ARS-OUR; and
    3. Referral slip to UPHS for Medical Certificate
  5. College SRE gives the Form 002 (College Admission Slip) to accepted applicant and two copies of Student Directory (to be accomplished by the student) to be forwarded to the ARS-OUR.
  6. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. College Admission Slip (Form 002)
    2. Two copies of accomplished Student Directory with photos
    3. Medical Certificate from the UPHS
    4. Photocopy of Birth Certificate, original to be presented
    5. Photocopy of Marriage Certificate, original to be presented (if married woman)
    6. Transcript used for evaluation duly stamped and signed by the Student Records Evaluator or College Secretary at the back of the TOR.
    7. Official Transcript of Records to be submitted in a sealed envelope
    8. Plan of Study
  7. Student submits the College copy of the University Admission Slip to the College SRE

  1. Student accomplishes application form and submits required documents to the Student Records Section on or before the deadline set by the college or school
  2. Submit the following requirements:
    • Application Form
    • Official Receipt of Application Fee
    • Official Transcript of Records
    • Photocopy of Birth Certificate, original to be presented
    • Photocopy of Marriage Certificate, original to be presented (if married woman)
    • Applicants whose medium of instruction in their previous school is NOT English should take and pass TOEFL with minimum score of 500 if paper-based, 173 if computer-based, or 61 if internet-based test or IELTS with minimum scores of 5.5
    • Other documents required by the college
  3. Student follow-up the result of evaluation on the date identified by the college or school
  4. College SRE informs the accepted applicant of the results and issues the following:
    1. Notice of Acceptance;
    2. List of requirements to be submitted to ARS-OUR; and
    3. Referral slip to UPHS for Medical Certificate
  5. College SRE gives the Form 002 (College Admission Slip) to accepted applicant and two copies of Student Directory (to be accomplished by the student) to be forwarded to the ARS-OUR
  6. ARS-OUR will issue an Acceptance Letter needed to get a student visa for the issuance of Study Permit from OSA
  7. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. College Admission Slip (Form 002)
    2. Two copies of accomplished Student Directory with photos
    3. Medical Certificate from the UPHS
    4. Photocopy of Birth Certificate, original to be presented
    5. Photocopy of Marriage Certificate, original to be presented (if married woman)
    6. Transcript used for evaluation duly stamped and signed by the Student Records Evaluator or College Secretary at the back of the TOR.
    7. Official Transcript of Records to be submitted in a sealed envelope/Authenticated copy of TOR
    8. Study permit from OSA
    9. Photocopy of passport, original to be presented
    10. Plan of Study
  8. Student submits the College copy of the University Admission Slip to the College SRE

Application to Cross Enrol (from Other Constituent Universities)

  1. Student submits approved Permit to Cross Register (PCR) to the University Registrar for approval.
  2. Present the approved PCR to ARS-OUR for issuance of referral slip to UPHS for Medical Certificate and list of requirements.
  3. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. Approved PCR
    2. Accomplished Student Directory with photo
    3. Medical Certificate from the UPHS

  1. Student submits approved Permit to Cross Register (PCR) to the University Registrar for approval.
  2. Present the approved PCR to ARS-OUR for issuance of referral slip to UPHS for Medical Certificate, accomplished application form and fee, and list of requirements.
  3. Student submits the following to the ARS-OUR for issuance of University Admission Slip:
    1. Approved PCR
    2. Accomplished Student Directory with photo
    3. Medical Certificate from the UPHS
    4. Accomplished application form and fee
    5. Study permit (for foreign applicants)

  1. Student goes to College for the issuance of College Re-admission Slip
  2. Submit the following requirements to ARS-OUR for issuance of University Admission Slip:

    2. Medical Certificate from UPHS (for LOA of more than one semester)
    3. Accomplished two student directory with photos
  3. Student submits the College copy of the University Admission Slip to the College SRE

Re-admission from Absence Without Leave (AWOL)

  1. Student goes to College for request for re-admission from AWOL.
  2. College SRE gives list of requirements.
  3. Submit the following requirements to ARS-OUR for issuance of University Admission Slip:
    1. College Admission Slip
    2. Medical Certificate from UPHS (for AWOL of more than one semester)
    3. Accomplished two student directory with photos
    4. Photocopy OVCAA Approved appeal for re-admission (for AWOL of more than one semester)
  4. Student submits the College copy of the University Admission Slip to the College SRE

COMPUTERIZED FRESHMAN REGISTRATION

  1. Confirmation of Decision to Enroll
    • You may confirm in person, via Internet, mail, or fax. However, you are strongly encouraged to confirm online.
  2. Pre-enlistment
    • You have to pre-enlist to have a better chance of getting the subjects and schedule of your choice. This is done online (via Internet) and involves choosing the subjects you want to enroll in and the corresponding time schedule
  3. Enrollment
    • Enrollment is done at the UP Diliman Campus. This includes payment of your matriculation fees. You are encouraged to enroll during the advance registration period to have a better chance of getting the subjects and schedule of your choice.
  4. Proceed to your College and get your registration materials
  5. Submit the following documents to ARS-Admission during enrollment :
    1. Fourth Year High School Card (Form 138)
    2. Secondary Permanent Record (if no Form 137, please ask for an official request to get your Form 137)
    3. Student Directory (2 copies)
    4. Notice of Admission
    5. Medical Certificate
    6. 2 recent photos (2x2)
    7. Study Permit from OSA (if foreign student)
    8. TOEFL result, if applicable(for students whose medium of instruction in previous school is not English) PB, IBT, CB or IELTS
    9. PSA-Birth Certificate (photocopy & present the original)

Freshman Admissions for Foreign and Filipino Applicants from Secondary Schools Abroad

A foreign OR Filipino applicant who has graduated or is graduating from a secondary school abroad (i.e. international applicant) and has not enrolled in college may be admitted to the freshman class by automatic admission OR by taking the UPCAT. For more information, you can visit this link

Note that programs are subject to availability of slots. Application period runs from June 1-30, 2017 (Filipinos) and April 15, 2017 (Foreign).

Automatic Admission

An international applicant may apply for automatic admission (i.e. without taking the UPCAT) if s/he meets the following requirements:

  1. Student accomplishes application form and submits required documents to the ARS-OUR on or before the deadline set by the University Registrar
  2. Submit the following requirements. Note that application period runs from June 1-30, 2017
    • Application form UP FORM 3A | UP FORM 3.1
    • Application fee $30 - Postal Money Order
    • SAT Result (minimum score of 620 in Math and 650 Evidenced BasedCritical Reading & Writing), or International Baccalaureate Diploma, or GCE Results (3 Ordinary and 2 Advanced Levels)
    • Official High School Transcript with date of graduation (Original Copy)
    • Applicants whose medium of instruction in their previous school is NOT English should take and pass TOEFL with a score of 500 if paper-based, 173 if computer-based, or 61 if internet-based test or IELTS with minimum score of 5.5
    • Birth certificate and passport
    • Affidavit of no collegiate enrolment (if with gap)

Application for UP Diliman Photo ID

  1. The following are entitled to have a UPD Photo ID:
    1. Officially enrolled UPD regular students and non-degree students;
    2. Officially enrolled UPDEPP regular students;
    3. Officially enrolled UPDEPO regular students;
    4. Officially enrolled UPIS students;
    5. Foreign Cross Registrants;
    6. Visiting Faculty and Research Fellows;
    7. Faculty, Administrative and REPS in active service ;
    8. Retired Permanent Employees (compulsory retirement)
  2. Student presents his current UP Form 5 and Official Receipt of payment (Php130) to ARS-OUR for photo ID processing.
  3. Employee submits approved HRDO application for ID form and official receipt of payment (Php130) to ARS-OUR for photo ID processing.
  4. Student goes to College Library for sticker/validation.

Application for Matriculation Refund

  1. The following are entitled to file for refund:
    1. Student was ill-advised;
    2. Student lacks the prerequisite for the subject;
    3. Student’s class schedules are in conflict;
    4. Student’s class was dissolved;
    5. Student with changes in their Socialized Tuition (ST) Discounts;
    6. Student who filed for an LOA or withdrawn his registration* .
    7. *Note: Before the opening of classes 100%; within one week from the opening of classes 80%; within the second, third, and fourth week from the opening of classes 50%; after the fourth week No refund.

  2. Student submits application form for refund to the ARS-OUR together with the other supporting documents like original UP Form 5, original student’s copy of COM, ST Certification, certification from the College (due to cancellation of subject).
  3. Student follow-up the status of his refund at the UPD Accounting Office.
  4. Student claims his cheque at the UPD Cash Office.

Application for Barangay Scholarship

  1. The following are entitled for a Barangay Scholarship:
    1. Dependents of a Barangay Officials;
    2. Barangay Officials;
  2. Student submits the following requirements at the ARS-OUR during the regular registration period for special assessment:
    1. Barangay Dependent:
      1. PSA Birth Certificate (must be a legitimate dependent child of an incumbent barangay official, 21 years old or below);
      2. Certification from COMELEC and Certificate of incumbency of the grantee’s parent barangay official from the Municipality/City Mayor and attested by the Local Government Operations Officer (LGOO) assigned in the city/municipality where the barangay is situated;
      3. Must carry a full semestral load as prescribed in the course curriculum; and
      4. True Copy of Grades (must maintain a passing grade).
    2. Barangay Official:
      1. Certification from COMELEC and Certificate of incumbency from the Municipal/City Mayor, and attested by the LGOO assigned in the city/municipality where the barangay is situated.
      2. For barangay officials, they shall enrol in SUC located within the province or city to which the barangay belongs.
      3. Must carry a full semestral load as prescribed in the course curriculum
      4. True Copy of Grades (must maintain a passing grade)

Application for Change of Name

  1. Student currently enrolled can apply for Change of Name.
  2. Student submits accomplished application for Change of Name to the ARS-OUR with attached requirements as follows (whichever is applicable):
    1. Original Copy of PSA-Birth Certificate, original to be presented;
    2. Original Copy of PSA-Marriage Certificate, original to be presented (for married woman);
    3. Affidavit of Discrepancy and/or affidavit of two disinterested persons;
  3. Student claims his approved copy of Change of Name at the ARS-OUR.

  1. Student applies for certification at the ARS-OUR and pay the amount of Php30 per copy.
